HNTB’s Northwest GIS Team created custom Survey123 Field Forms to assess streams with regard to current site conditions, how best to divert the stream and exclude fish, and use Washington State Department of Fish and Wildlife (WDFW) Site Description Field forms. With Washington State Department of Transportation (WSDOT) evaluated current stream, culvert and surrounding environment regarding fish exclusion and stream diversion for culvert replacement projects. Using Survey 123, standardized observations for WSDOT and for forms submitted to WDWF, and prepared template formats for standardized report exports. These forms provide a single-source to survey data, print standardized forms, and the web-first approach ensures the use of consistent results and formatted reports. This presentation describes the use of this workflow for ease of data review and how to use online-forms for web-first data analysis and reporting.
